Spring 2007 Course Requests

This form will be used to select the courses to be scheduled for the next semester. You should use the form to indicate 4 courses which you would like to see offered and for which you wish to register for the next semester.

Please select 4 courses from the following list and in the box next to these courses type a number corresponding to your preference. Give a score of 4 to the course which you most desire to see offered; this is the course that is most important to you. Next enter a score of 3 to your next preference, a 2 to the next, and a score of 1 to the course that you would like to register for but is not as much of a priority as the other ones.

If you are planning to graduate next semester and any of the 4 courses that you request is needed for your graduation, then please check the corresponding checkbox in the third column. Then hit the submit button.
