insurance carrier for the plan year beginning=20
July 1, 2008 must complete their change document=20
no later than Wednesday, April 30th at 4:30=20
p.m. No carrier changes will be accepted after this date.

Since the Human Resources Office is temporarily=20
relocating for renovations the week of April=20
28th, if you plan on making changes to your=20
health carrier or bringing in dependent=20
verifications, please call in advance so that we=20
can ensure that you are directed to the correct=20
location. The contact numbers are 2057, 5451 or 5655.

As an additional reminder =96 Dependent Verification for
PPO, Humana =96 HMO, United HealthCare - EPO
and Dependent Life Insurance

During April 2008, employees enrolled in Health=20
Insurance and Dependent Life Insurance have an=20
opportunity to voluntarily drop ineligible=20
dependents from coverage with no questions asked and no penalties.

Between now and November 30, 2008, all employees=20
MUST present appropriate written verification for=20
all currently covered dependents to their agency.=20
Retirees must mail documents directly to=20
OGB. Coverage for unverified dependents will be=20
terminated effective December 31, 2008.
