PHI KAPPA PHI LOU BALLARD ENDOWED SCHOLARSHIP APPLICATION
Deadline: 4:00 p.m. Monday, April 12, 2012

1. In order to qualify, students must meet the following criteria:

a. Be a full-time undergraduate student at the time of selection and during the semesters he or she receives the scholarship

b. Have a minimum 3.5 GPA and be an active member of the Southeastern Chapter of Phi Kappa Phi.

c. Be of moral character that exemplifies the spirit and intent of Phi Kappa Phi.

Complete an application from the Development Office, verifying GPA and class status.

d. Submit two letters of recommendation from faculty members, one faculty member from within the student’s major and one from a discipline outside the student’s major.

e. Submit a personal letter explaining his or her plans for a profession or career, his or her gifts and talents that can be used in this field, why the applicant chose this field, and what he or she hopes to contribute to the field.

f. Include a copy of his or her resume.

2. The scholarship recipient will be announced during the Phi Kappa Phi Spring Banquet (Tuesday, April 24, 2012). The scholarship will be awarded on a semester-by-semester basis, $250 per semester. The recipient may retain the scholarship for two semesters provided he or she continues to meet the selection criteria.
